Does anyone know who may be responsible for a driveway that has been damaged by the placement of a JEA water Meter? The meter was installer about 10 years ago and sits in the middle of our cement driveway and has had a crack from the meter to the street for years. We called them and mentioned we were concerned about it worsening and damaging the meter. They came out a year ago and placed an ugly, noisy metal plate
over it. Now it has cracked even more and is caving in.
Has anyone had an issue like this?

You're likely responsible for it, considering it was probably caused by settling of the ground underneath the concrete.
