Leave it off. Contact the prior employer and state clearly that they do not have your permission, expressly or implied, to share any details about your employment beyond stating the time period during which you worked there.

If you have pre-paid legal, you can ask them to draw up a letter. If they state anything about your employment, even honest things, against your explicit instruction to ****, the law will be on your side. Of course, it might not be practical to sue, but the thought that you might can be enough to entice them to be silent. If you did sue, the manager you named would hear about it and probably take a ton of flack from their boss.

Did you get a reference letter from the last job stating the dates of your employment, your title and such? I would contact HR and say you have a job offer and they are doing a background check and what the new job HR will expect. Most companies just want to verify that you were employed there. If the company you got the job offer ask why you left, say the same thing HR says. They will probably say you were terminated due to unforeseen circumstances. You can elaborate saying that you got a job offer.

*UPDATE*

So, I received one phone call yesterday about a wrong phone number to a temp agency I had listed. But I passed the background check! Whew! I was a little worried there!
