That would be Primanti Bros - as the story goes, a lot of blue collar workers would stop in & need quick nourishment. So, the sandwich became the whole meal all at once, which would allow them to use only their hands to eat & satiate them quickly.
There's a place in Chicago near Wrigley Field called Lucky's Sandwich Company; they've basically copied the idea. I've tried it there; while it wasn't the greatest, it was interesting & fun to try. Certainly a novel approach. Most people don't realize until they get there that the cole slaw & fries are ON the sandwich, not side dishes.
