Hey guys! I have a bit of a complicated issue here, so bear with me.
I was naturalized in 2001 in LA. I changed my legal name when I got naturalized (I originally had only my Chinese name on my Green Card and Social Security card). To my understanding, the name change request had to go to a California court - then the Certificate of Name Change was supposed to be mailed to me along with my passport.
This is where things got complicated - I told the passport agency to mail the Name Change paper and the passport to my university dormitory (which was up in Berkeley), and they mailed it to the wrong address, so that package was lost.
I promptly applied for a replacement passport at the Passport Agency in San Francisco, but I had no idea how to order a replacement Name Change paper, which has created a lot of issues for me since that time (I can't correct my name on my Driver's License or my Social Security, because they both require my Name Change form)
I tried to Google the issue but I simply can't find a straight answer to my problem (because most results for "Name change" applies to people who went through the normal name-change process at a local court - and I didn't go through that, because the request for name change was sent to a court on my behalf by the passport agency, and I don't even know which court approved/issued my name change)
What should I do? Thanks very much in advance to anyone who can help me with this matter.
