They probably just hopped on the bandwagon during the late 1960's.

Depends on your definition of 'rank and file' How low on the structure of employees do you want to go? Looking at IT people as an example. The one's that perform the actual work of answering job tickets, setting up and maintaining the networks ARE union at the LIRR, but they are NOT union at Metro-North.

Are they "independent contractors" or working through agencies and not directly for M-N aka "permatemps" with no benefits, etc.?

I don't like THAT either. But too often that is what happens as a result of corporate greed and also as a result of over-entitled unions scaring the crap out of employers so bad that they don't even want employees on their books at all.

I like a happy medium. Not too much power and resultant abuse on either the employer or employee side. Why is that so hard?

No, they are actual MNR employees, with the usual benefits.
