My child is not doing well in school and I request from the guidance to receive a weekly progress report, so that I can be on top of it. I was told I can receive it once for a 6 week period. They were very adamant that it could not be done throughout the school year. My question is how can I apply for 504 services? I had my child diagnose in 3rd grade for ADD and there was no problem. He is now in 7th grade.
You need to speak to the doctor that is treating your child for their ADD. Have you spoken to the teachers? Is it possible to be able to call them directly for updates for your child?
I do not work on Long Island, but in my school in NYC the ADD/ ADHD students who recieve 504 services generally recieve extra time or front seating, I've never had a 504 for progress reports. Typically, you need to show that the requested accommodation is needed for your child to have the same education as a child who does not have the same medical condition as your child.
If you have a good teacher or teachers alot of times they will oblige you. My son has ADD and his teachers give weekly reports and if he has an issue for a specific day, they send a note that day.
Generally, your child does need to be classified in order to receive services. As a parent, you have the right to request a re-evaluation. You also get to choose the evaluator. If you keep getting denied services, you can hire an educational lawyer or special ed advocate. NYC dept. of ed does things differently from the rest of the state. I would also suggest contacting the president of your school's SEPTA to get the inside scoop on your district's special ed services and which adminstrators and teachers would be the most helpful. If you want services, you really have to be a persistent nag (in a polite way) with some districts.
The squeaky wheel gets the oil. You MUST attend the next SEPTA meeting in your child's school and ask questions. Be polite but be firm.The district is going to want you to go away but persistence pays off.
I believe the 504 and the weekly updates are separate issues. As far as a weekly progress report, I'm sure if you email the teachers on a weekly basis, they will reply back to you. The students in my classes who get a weekly progress report typically lose them or forget to get them filled out. A email from the parent weekly has always been more reliable. The 504 process is a lot more difficult these days. Good luck.
You need to call your neurologist about getting a re-evaluation or updated report-I dont know if a school will give one-504- based on a 4 year old report. I would ask the dr to get involved and call the school as well. You can call the school psychologist and begin the process. Let him/her know that this is where you are going-she can help set up a meeting with you and the teachers to figure out the best accommodations for your child.
Does your child have an IEP? If not, you would need to have him/her evualated by the district and if needed, they will come up with a plan. Make sure you go to all the meetings for your child and let them know you are 150% behind your child and you will not let anything slip.
