I'm assuming you mean an actual nursing home as opposed to assisted living. Personally, I wouldn't send anyone to a NH unless they were on a ventilator. I've never heard anyone rave about a nursing home. Cold Spring Hill, while in a beautiful location, is absolutely disgusting. It's very large and old. There are rooms with 4 patients each, and the beds look like something from the 3rd world. Parker Jewish seems decent. It's clean and they do have activities for the residents. It's also right next to LIJ. Good luck.
I have worked in many nursing homes here on LI and most are pretty good. Yes, Cold Spring Hills is a DUMP! Others I have not liked were Avalon Gardens in Smithtown and East Neck. I have heard Petite Fleur is just okay.I have liked St Johnland, Huntington Hills, Good Samaritan in Sayville (very nice, small and homey with personalized care and good activities and even good food), Nesconset (great rehab), St Catherine of Siena in Smithtown (especially good for cardiac), Our Lady of Consolation in West Islip, Woodbury Center (haven't been there in a few years but it was good.
As an independent contractor, I have easily been to 50 nursing homes on LI and the only ones I would run screaming from are Cold Spring Hills and A Holly Patterson (just from past reputation alone).
Oh yeah, and Gurwin IS great! Like others mentioned, the Dept of Health does annual inspections of every nursing home and the results are made available to the public.
I too have quite a bit of experience in the nursing homes and rehab centers around the island and I would have to agree that some of the best are Gurwin, Nesconset Center, Good Sam in Sayville. All are excellent facilities. San Simeon in Greenpoint is also very nice.
I would steer clear of Cold Spring Hills, Avalon Gardens, Brookhaven Health Care Facility, and Peconic Bay.
