My husband was born at Mercy in 1960 and his birth certificate says RVC. I seriously doubt the location changed over the course of a few years, if you can apply for Medicare you are not much older than him.
About to apply for Medicare and I need to provide not only the State (NY) but city I was born in.
Are you sure they need the city of birth? (Actually there are only two cities in Nassau County and other locations are towns, villages and hamlets.) Looking at the Medicare Part A application form it only asks for the state. Where were you asked for city of birth? Are you using a firm or individual to enroll you in Medicare? The government doesn't seem to require it.
You need city for Social Security, and getting it wrong will delay approval of application and require you to go to the office with proof to correct it - the city information was gathered when your SS card was issued, and the government uses it to verify identity and prevent identity theft and other post-Patriot Act concerns. The data in the Medicare database for you is built from your SS data.
I remember applying for medicare and I am certain that I brought a certified copy of my birth certificate. There are alternatives, but I am certain that just someone "telling" you that info isn't one of them.
