Next year I get out of the Air Force and want to come back east to put my kids closer to the grandparents. My wife's dad lives up in Elkton, MD and while I've always liked the Eastern Shore, I'm not sure what it's like to actually live there. What's the situation like there for docs? I've heard the new Maryland Governor seems to think the best way to raise revenue is to tax people to death, so I'm not sure if I should be looking in Maryland or broaden my job search to Delaware or Virginia.

I don't know the situation for your jobsearch but VA is VERY expensive! So is anything close to DC.

MD is sometimes called "little America" I was told. Because it has a little of everything except the desert. There is urban living and rural living, there are mountains, rolling hills, woodlands, wetlands, lakes, the bay and the ocean all in one little state. You have beach weather in summer and snow in the winter. There are many things to do and places to go. I would say come and visit and just drive around, all over. When you find your type of place check out your job options or vice versa.
There is state income tax. The sales tax is 6% but you can always drive to Delaware for no sales tax purchases. I think on a national level the property tax is not bad if you were to buy a house.

The hospitals in Easton and Cambridge merged with the University of MD Medical system a year or two ago and they claim to have a shortage of doctors in the area. Also they are planning on building a new hospital just north of Easton in the next few years. When I needed a new doctor I found it a little hard to find one accepting new patients.
