I really need help. I have been a correctional officer for several years now. 4 months ago i almost took my own life and the counselor and the nurse practitioner wrote a note for me. Once i came back i while i was looking out the window and i saw alot of the buildings with inmates i was shook up. i was questioning myself can i rally handle this job. And then the supervisors advised if i am ready and i told i believe so and then they thought about and then they me lead to the the wardens office and i let everything from there. Now I have to go back the doctors office soon and I have to fill something out where it goes downtown and then if they notice that i cannot work on a unit ( depends on my medical needs) if they cannot assist with my medical needs then i believe they can fire me I dunno.
So what I am asking what should i write down the papers about my medical needs. i do suffer from anxiety attacks and major depression. I want to put down something where they look at it and they see that i can work in an office.

Ask your counselor, who should have experience with this. You shouldn't have to be dealing with this by yourself.
Do you work for a municipal government ( city, county, state )? If you do, then you most likely belong to a union. You should discuss your situation not only with your doctors but with your union rep.
You' have to go over your employment contract with your union rep and see what it says about your ability to perform the duties for which you were hired. You need to see if it can be interpreted that your issues are stress related to your job or not. If it's work related they may have to accommodate you.
This is both a medical problem and a labor relations issue. You're going to have to get really good advice on what they can do with your employment.
I belive you are correct that if you cannot do your job that they can let you go, even if it's due to a disability if you still can't do it with reasonable accomodations. It is unclear what you mean by "cannot assist you with your medical needs", but I assume it means providing reasonable accomodations; those can include putting you in a different position that you are able to do. Someone else mentioned that you are most likely in a union, if so, speak to the uinion rep and also inquire as to what type of long-term disability insurance you might have.
I do genuinely sympathize with you, I have struggled with depression in the past and also feared this happening (also in law enforcement). It's not usual for people in the field to experience mental health problems themselves, but there comes a point where that can interfere with duties. I think you already understand that you likely won't be permitted to return to work directly with offenders after what has happened, but office work sounds like a wise idea. Is there a union you are dealing with? What about any mental health programs for employees?

Best wishes for continued healing Hourglass. But to do that, you are going to have to find a new career field I think.
Don't try to stay with a job that you know isn't right for you! Do what you have to do to get where you know you are meant to be
You did nothing wrong and I am sorry the system is screwing you. That said be very careful about what you say to any psych going forward. Ask what their reporting requirements are first.
I am sorry but your careeer is probably over working with offenders of any kind.
