I am so sorry to hear you are having to deal with this mess, I can only imagine how frustrated you are. If any of the services the Orthodontist preformed was covered by insurance, you can go to the insurance company and see if they help with the situation. I know when I was dealing with orthodontic services I had to sign a treatment/services plan that spelled out what the treatment plan was and the costs of services, if you also entered into such a contract you can sue the orthodontist in small claims court. (easy to do pro se, you do not have to hire a lawyer)
I'd also recommend contacting the state licensing board...it may be that he left the practice because his license was lifted. (considering the shoddy work you report)
I am not sure where you are reporting this guy, but you should definitely report him to the State Board of Dentistry (a division of the NJ dept of Consumer affairs) here is their website:
