You are disqualified for benefits from 07/21/13
you left work voluntarily on 07/21/13
you were on a medical leave and you provided a doctors note that
You were exvused from work untill 08/05/13 , you had to return to
Work on the following day and failed to return to work . Your failure to
Return to work when the doctor released you constitutes a voluntary leaving.
Of work . You lefy your job without good cause cause of attributable to the work .
You are dis qualified for benefits .

How can i appeal this? my supervisor says i.quitvoluntary ?

Why would you listen to a co-worker and not your boss? You should have taken your butt to work the day that you were supposed to.

people do this because they see an opportunity to collect unemployment and sit on their butts not working.

In reading that thread, it sounds like the OP needed to be on short-term disability, not unemployment, in the first place.

It sounds like you tried to collect unemployment while you still had a job - that's why your employer thought you quit, because unemployment called them to verify that you used to work for him but didn't now....you needed to apply for disability, you screwed yourself by applying for unemployment instead. If you appeal this, chances are they will make you pay back for the period you were actually disabled and not looking for work.
