revolvers don't "jam" per se so I think there must have been some kind of misreporting

Revolver jams are rare but they can happen. I had a s&w 686 jam when a bit of oil was left on the extractor rod in the back of the cylinder. Powder build up in the oil kept the rod from seating flat . When the trigger was pulled the cylinder would not rotate and fire.
