I'll add another vote for Jacques Torres. It's NYC based so probably not likely to be found elsewhere. One thing I really enjoy is their hot chocolate. You can buy the mix (I like the spicy one) and it should be something that's easy to travel with.
I like other places more for truffles but they don't keep for more than a few days.
Whole Foods generally has a good selection of chocolates including some local producers if you want to look into that.

The best chocolate available in NY is not American, other than Torres stuff. Hersheys is awful - I can' believe people have been duped into buying that waxy butyric acidy crap for years. Maison du Chocolat, as the name suggests, its actually French. Many delis do good British or Swiss stuff - but its British or Swiss. Homegrown good US chocolate? Torres, or perhaps Lil Lac.

franchisewise, rocky mountain chocolate is pretty good.
If i'm at the supermarket, I usually get ghiradeli 72% to 60% dark chocolate squares
