I also recommend breather.com. Used a few times and it worked great. WARNING THOUGH: They will charge your debit/credit card a pre-authorization / TEST FEE of like $250 or something, which will be returned to you (after a few business days) -- just to TEST your card to make sure there's enough funds. It is so annoying. When I first signed up, I was horrified and started freaking out immediately thinking that I've been scammed. But it's a legitimate thing.

Maybe better is the SAKS bathrooms. Stalls are floor to ceiling mahogany, Quiet as a casket.

Assuming you can't reschedule the doctor appointment, why not ask the doctor's office staff if they can provide a quiet room?

I once had to do a phone interview while in midtown. I went to Bryant park thinking it would be somewhat quiet. I couldn't believe how many sirens and honking cars went by during the call. Just tried to laugh it off and told the interviewer it's hard to find a quiet spot in nyc. Still did manage to get a job offer after that.
