First time I've seen that happen. I told the bus driver and he told me to complain to the MTA. Is it worth the trouble?
It's a new metrocard so I hope there isn't something wrong with it.
Also another thing I've noticed I took the 1 train from Christopher to 23rd/7th back in Aug. and stopped at dunkin donuts before catching the 23rd Street Crosstown when I boarded the crosstown bus and dipped my card it deducted another Fare!!! ..It was only maybe 1/2hour between when 1st swiped to catch the 1 and when I got on the 23....I was tooo much in a Rush to the East Side to Argue it with the Driver, but I am noticing a pattern with the reliability of the MetroCard system...
Was there more than a 2 hour gap between when you started your trip and when you transferred subway to bus?
No. Not even close. I know the rules. The charge was simply wrong.
I'll assume it was just a fluke. The driver seemed to think it had something to do with the fare box. But I didn't see anybody else complaining (unless people just don't watch to see if they're being charged when they shouldn't be).
I don't think it's worth a 44c stamp, an envelope, and the time to explain yourself to send a Metrocard back to MTA to recover $1.96 (the effective cost of a $2.25 fare with the 15% bonus).
I've had that happen to me as well a few times. If you wanna go through the trouble, there is a process where you can contact MTA and they send you another metrocard. I did this a while back when I had a metrocard for like $20 that went defective on me but I don't remember all the details.

I believe there's an office to handle MetroCard issues at 3 Whitehall Street (just up from the Staten Island Ferry terminal in lower Manhattan). You might try going there and dealing with things directly.
