You can't really go wrong by being kind as long as you don't want something in return. Letting the doctor know that you will recommend him to others will keep it business-like.

Sounds like a great idea, and you are correct, they don't get thanked enough. My husband is a surgeon and occasionally gets cards and small gifts from patients. If they make it home, I try and keep them in a file.
For every nice person like you who takes the time to send a card, providers (and their offices) are dealing with 5-10 other people who are unappreciative, rude and often noncompliant in their treatment. So, yes, sending a card to remind them of the good work they do is very appropriate.

A few years ago I was on life support........one of my doctors that was not part of my treating doctors heard about it.
I got a card in the mail.
It was from Habitat for Humanity.
He had made a contribution in my honor.
I will keep this card forever.
