"Worst" county is subjective, Hamilton County must be bad if so many of the people that work there live in Warren County, same for Montgomery County and the number of people who work there living in Greene County, also Franklin County and the number of people commuting to Delaware County.

Vinton County made a lot of the "bad" lists, but if you like living in isolated hill country and McArthur is all the big city you need then it could be a utopia. My favorite part of Ohio is the South East section purely for the unspoiled countryside.
