As far as the waiver forms go would you want to let some one know if your child had allergies or had specail needs.
It is data form for those purposes only.
Stop the maddess people. I enjoyed the party and the hostesses were great , the concept is great.
The pictures were only of the party setup and the girls hands and feet.
No harm at all.

It's a Glitz and Glamour Party not a Hooker and Ho Party.

I doubt make-up will be used. The girls have their nails painted, choice of hairstyle and dress-up clothes and accessories to choose from. I see no crime in letting little girls have fun like this.

Let her go and if she comes looking over the top, you have the option of fixing the problem.

Have a story to relate about my own glitz and glam party way back in the 60's. I don't think they were called glitz and glam but same idea. I was probably 9 or 10 and came back with full makeup and a beehive hair-do. When I came home, my poor dad about had a heart attack since according to my mom, I looked like a miniture hooker. I had to immediately wash it all off after my poor dad recovered. I still giggle a little bit thinking about a beehive hairdo on little old me.

I'm sure that today's glitz and glam parties are a bit more.....tasteful? A nice hairdo and some nail polish won't kill the kid.

I feel bad for the daughter of the OP.

Reading some of the posts on here I think these parents are TRYING to ruin their kid's social lives. Light up, good god.
