i have recently had the need to find a few lawyers for probate and Estate planning i began my search on Avvo

if you have some questions about a legal matter you can get a lot of free answers at this site. Yes the lawyers are looking for clients but it gives you an opportunity to judge the knowledge and tone of the responses and begin investigating them. Call a few, most offer free consults, and judge your instincts. Were you able to talk to them quickly, did they sound knowledgeable, remember you have no obligation towards them, so call several. Frankly even if you don't need a lawyer now its very helpful to have knowledge of a few in advance of any issues. It makes stressful times much easier to deal with if you don't have to find a lawyer at the last minute.

What really matters is what the "neglect" is, the purpose of their visit to this establishment (are they paying customers--i.e. business invitees), and what the actual injuries are. If the injuries are feelings hurt and increased blood pressure, then this is nothing. If there is an actual injury and the neglect was within their property, then that's a different story.
By "official" I meant disinterested, unbiased. If it's not a large company, but rather a local restaurant, shop or other business, I would not be surprised if there is no written record at all, and no witness information or statements. The OP would have been wise to get witness contact info herself, but was too busy trying to call a lawyer. The OP didn't give us enough details, and apparently won't be back to provide them. So, we will never know.
