Technically speaking, did he violate the TOS here?

Generally, sockpuppeting (setting up a second account to fool people) is an instaban on most forums. But merely renaming yourself and pretending you're someone else is just...weird.

The web has created a whole new coward, but again am proud to use my full name always as my screen name.

It’s easy to spew your opinions and feelings under some ridiculous nick name, but it takes a real adult to do the same and sign their real name. Make fun of specific people in the press all you want, but they aren’t hiding behind some moniker.

Defeating the auto-censor by p l a c i n g spaces between the letters of a forbidden word is probably frowned upon. As far as the name change goes, that would need to be done by a Forum Administrator, so it would have been done with help from the staff. Just ask Scan Barre, er, Reston Runner, er, Steel City Rising...
