We've seen examples of:

original long form BC's

newly printed laser printer versions of COLB's

Have we seen a short form COLB as it was issued in 1961 by Hawaii?

no, because such a document did not exist at that time. birth certificate formats change over time. as far as i know this is true for all states. the hawaiian COLB still meets all the requirements of the US state department for proof of birth.

wrench, what state were you born in ? what would your state release if you said you lost your original BC?

I was born in Illinois. What they'd issue now - I don't know. I'm never going back. Plus I still have the original photostat copy given to my parents when I was born. In all my wanderings all over in my lifetime, hurricanes, tornadoes, fires, burglaries, etc. I still have it. It was important enough to keep safe.

well, you are a better man than i gungha din. twice in my lifetime i thought i lost my BC when i needed it but found it at the last minute,
