I agree with jojajn that it was the orchestra that was the bigger screw up. Even IF protocol was as you say, the orchestra leader should have adjusted the start time of playing 'God Save the Queen' to keep from embarrassing a head of state who was giving a toast.
The orchestra screwed up but you Obama haters will of course never believe that.
