I'll start with the early mistakes:

1. Encouraging snitching flag @ whitehouse . gov
2. Promising to close Gitmo before he assessed the situation then had to backtrack.
3. Having no leadership/managerial experience and then making a bunch of academics, also with no real world experience, his top advisors and you wonder why the economy is still in the toilet.
4. Cronyism- Sucking up to the Unions and badmouthing the Chamber of Commerce
5. Having a tax cheat run the Treasury Department which is the department that the IRS is under.
6. Naming a series of people with tax problems for advisors sending the message the tax laws are just for us regular people not for the political elite. Their tax problems aren't his fault but he could have vetted them better. Oh, and then there was the self avowed Communist who was exposed and his dubious friends (Chairman Mao, mass murderer, is my hero) in White House jobs.
7. Signing Spendapalooza also called The American Recovery and Reinvestment Act when it was full of pork.
8. Putting Obamacare and trying to put Cap and Trade ahead of the economy/jobs when clearly Obamacare and cap and Trade were not the primary concern of Americans
9. Cash for Clunkers with its 135 pages of rules for dealers - 41 percent of cars sold were from Japanese makers.
10. Bailouts for fat cats - Tough nouggies to small business owners.
11. Sending the Winston Churchill bust back to England, giving DVDs to Gordon Brown, giving the Queen an IPod with his speeches, bowing everywhere he went, dissing Israel, an ally, the Olympic fiasco, telling France they were our "best friend" much to the chagrin of Great Britain
12. Civilian trials for terrorists so they can lawyer up and clam up. Attempted to try terrorists in NYC with his Attorney General saying on the one hand we need to show the Middle East how fair the American Justice System is while in the next breath saying even if they are found not guilty we're not releasing them.
13. Promising money to Petrobras (Brazil) to do the same kind of deep sea drilling he disdained being done off the Gulf Coast making it look like he was deliberately trying to impale the US economy.
14. Never studied the economic impact the drilling moratorium would have in the Gulf or the impact on oil production (his Undersecretary of Commerce testified). Tried to pass off the moratorium as a recommendation by experts in a report but the experts made no such recommendation. His administration inserted it after the experts had submitted their report. For months his administration failed to issue drilling permits in the Gulf until it was exposed and there was a public outcry.
15. Saying the border was secure when anyone with half a brain could see from video that it clearly wasn't.
16. Closed door meetings while spouting transparency claims.
17. The reason he had to have the beer in the garden photo op was because he inserted himself in something that was none of his business.
18. The massive expansion of the federal government under his administration.
19. Creates a debt commission (Bowles/Simpson) then ignores their recommendations.
20. With Democrat control of the White House, the Senate and the House of Representatives failed to pass a budget in 2010.

He failed to close gitmo and end the war in Iraq when he said he would. He's also failed to secure Afghanistan to a level where we can leave without major issues.

MISTAKES

I'd say that the wall street bailouts were a failure. Not because of the bailouts themselves, but how they were used.

RECORDS

1. Record home foreclosures
2. Record number of people on welfare and receiving food stamps.

Home foreclosures are to be expected when you have a bubble burst.

BROKEN PROMISES

1. Closing of Gitmo
