You don't work in health care, do you? Medicaid already reimburses at rock bottom rates (the lowest rates in the industry) to the point that it COSTS doctors and hospitals to treat medicaid patients. Hence the reason so many doctors and hospitals are no longer accepting them. The way the system is set up, providers are not allowed to charge the patient the difference. The patients cannot be billed for co-pays or deductibles. How can you incentivize healthy lifestyles when they are getting this benefit at no cost already? What "incentive" could you possibly give someone?

I think you're confused. Medicare is for seniors, Medicaid is for poor regardless of age. Sometimes, people qualify for both.
