Historically, all risks were mutualized within group policies. That's been changing, too.
You could pretty easily prove it via web browser history, among other things.

The real answer is that smokers are a minority, only about 20% of the population. If you go after the promiscuous, the overeaters, and the drinkers, you're talking a much bigger voting block. It's exactly the same dynamic that kept blacks as second class citizens for most of US history. They were never more than 10-15 percent of US population, so they were easy prey for the haters.

Oh, right. What I meant to say is: Republicans don't want poor women getting contraception, and they don't want kids getting contraception without their parents' permission, and they don't want ANY woman having access to the morning-after pill, and they don't want kids to be taught that contraception exists.

Must be a small group. Most large groups have one rate for all employees. That is changing, but still isn't like individual insurance.

When I was looking for a policy (I'm self-employed) the company I applied to came to my house and did a physical, which included testing for nicotine. I was a smoker at the time (I've since quit) and my broker warned me they would test. I tried not to smoke for a few days, but as anyone who is addicted to nicotine knows, that's really hard. I failed that part of the physical and my higher rate reflected that.
