Yes, the Community Health Alliance (linked in the Daily Caller article), Tennessee's health insurance co-op, is doing the promotion with their own money. No other group is doing it. It is not a bribe, as they say it is to:

The idea is to make it easier for providers and patients to stay connected, but it will also help CHA keep track of its member population, many of whom are expected to be new to the health insurance market.
It will have health apps, PCP contact information, and directions to urgent care centers. The phone comes pre-loaded with health tools to improve the members health, which must be used as part of the contract. They must see a doctor as part of the agreement and work with a health plan (like stopping smoking, and/or getting fit) to improve their health in order to get the phone for free.
Oh, the horror of a group using their own money to help people.
