No, corporations do not have to compete to survive, in fact they can improve their bottom line by merging into monopolies and cabals for the purpose of fixing prices. That's where the money is.

Your right on the Death Panel part for reasons unknown Republicans wanted businesses to set up Death Panels for Ryan Care rather than have a government Death Panel. The bottom line though with Medicare reform is that way too many Republicans are on Medicare for them to actually do any meaningful reform without bipartisan cooperation.

It the state of Kentucky wanted to implement a single-payer system nothing prevented them from doing that. Ironically the ACA effectively killed state systems. A recent article seemed to indicate that the system in MA may not comply with the ACA. We have heard nothing but praise about their system (Romney care) and now it may not comply.
Actually the ACA has provisions that allow for states to implement innovative systems so long as they meet the same minimum standards of coverage. Vermont is still moving forward with putting together single payer through Green Mountain Care. Kentucky, or any other state could do the same if they were so inclined.
