The ex-CEO of United Healthcare received 1.8 billion in severance pay, so it's not out of the realm of possibility, at least for him, but if suddenly you were in possession on that amount of fiat currency, what would you do? (to coin a phrase by John Quinones).
That actually is a pretty hard question. I think first I'd buy a nice place in NYC or Boston. Then, I'd buy whatever else I felt like I needed or really wanted. But I would mainly donate it all to charities who I knew would make good use of my money. I'd try and make a difference in this world.
.It is relevant at this point to note the compensation package of William McGuire MD, the CEO of United Health Group (the parent company of United HealthCare) who was ousted last year in a stock scandal.
McGuire received hundreds of millions of dollars in compensation from United, has a severance package valued at more than 50 million dollars, and has stock options valued at 1.8 Billion dollars .
Besides being obscene, it is hard to reconcile McGuire's pay with the UnitedHealthcare mission statement : "to improve access, promote quality, and make health care more affordable."

I don't even know what a billion dollars is, but for the sake of having some fun:
* Remodel my house
* Buy some nice clothes
* Pay off all debt
* Take some vacations and treat others
* Host some pricey dinners out
* Give each of my kids $400k
* Buy a few things I need (i.e., computer, tablet, printer, etc.)
* Give to specific individuals in need (in creative ways . . . this would take some thought, but I would want it to be personal)
* Create a non-profit business that would do some good in the world (have no idea what but I'm sure I could think of something)

Good grief, no wonder your health insurance is expensive! I'd pay the taxes, then give it all away, every cent.
