They are insurance policies just like the ones people buy outside the exchanges.
Along with others, I posted links to articles reporting that reimbursement rates are lower for exchange plans. From the Kaiser Health News ---

Dr. David Martin did not want to answer any questions. Blue Cross Blue Shield says his reimbursement rate would be the same regardless of where the plan is purchased so Julie thinks this is a political statement.

I am showing my naivete here because I have never heard of a $250 deductible. That's almost dollar one coverage!
Seems like the premium would have to be astronomical or heavily subsidized in some way. Or perhaps there is extremely careful screening of the insureds to verify that they don't have anything wrong with them or a history of getting sick.
I have a $200 deductible...have for years.
