No, she would not need one.
It is not an actual position that needs to be filled. The term "First Lady" wasn't used until Buchanan, because he was unmarried so his niece would host the events for him. Our culture has moved past the point of needing a female hostess to pamper the guests...at least I hope it has.
Bill Clinton could be the "First Gentleman," except too many women have accused him of rape, assault, and harassment for him to have that title in an honorable manner.
Given his past and considerable failures as a responsible husband ... when Hillary is President one of the challenges will be keeping Bill away from the help, and his zipper in the up position.
A first lady would be a big help, that's for sure. The thing is, women have always had to work in the business world w/o these little helps that men have. For example, of the doctors I work with, four are men, one a woman. The men have these wives who drive the kids to all their activities, they all send their kids to private schools that provide no transportation. The woman has a husband who works, her kids go to public school!
Hillary could appoint her daughter to the First Lady role or break new ground and hire a social secretary/event planner that takes care of events at the White House. I don't see Bill doing it. I think he would continue with his foundation work. Like Katrina suggested above, that's what happens in the real world when a woman has a high power career and it's what Bill was doing while Hillary was Sect. of State.
