That is because the British system is socialized, doctors, nurses, hospitals etc. Most european nations have a mixture of the two but nothing like ours.

The British System involves private health providers who carry out contracted out services, whilst NHS Trusts and Teaching Hospitals run their own budgets.

Lookup "Advantage Plans- Supplemental Plan" There are other premium paying plans that piggyback Medicare.

"Elective" meaning total hips, knees, and spine surgery without foot drop.

That is not a choice, it's an add on, much like what is going on in many European countries, you pay high taxes for single payer and then pay even more for private insurance to make up for how crappy the single payer is.

How would those hospitals pay for the 2 million dollar MRI machine?
Your premise is wrong. Facts are, all developed nations, of which many are much older civilizations than ours, have all developed single payer, universal health care systems for their citizens, rather than a free market system, like ours.
