Yes, society does determine it. Even in communist ones. You think some worker comes before a high level politician in those countries?

Societal structures are made up. "Worth" is an opinion in societies, not based off fact.

Worth only exists in the context of societies. Afterall, you're asking society to control who has access to healthcare. Guess what.
