The polite way of putting that is "humans are error-prone in the absence of a well-designed and rigorously implemented procedure".

Even less likely than hacking the destroyer. These things have no maneuverability. I could dodge one on 10 foot sailboat in low wind. The destroyer is a ballet dancer in comparison.

Popular press nonsense. Most of these systems are local and have no outside access. Some may be remotely writable but if you are remotely worried about it you don't allow it or only under very stringent conditions. Protecting big systems like Amazon or Google or even a big web site is tough. Air tight protection of a throttle control system is not.
