Sure, if the person performing the service decides that is what is best, but when politicians and bureaucrats forcibly make healh care decisions, thats where the problems start.

I like those whose unhealthy lifestyles to be charged more though like car insurance

Untreated gum disease or tooth decay. You do not want to go into an elective surgery with a pre-existing infection.

A friend of mine had to wait nearly six months for his hip replacement in order to clear up his gingivitis and get a clean bill of health from a dentist.

You think insurance companies aren't a factor in these decisions? A surgeon who doesn't pay attention to this stuff could get his malpractice insurance yanked real fast. Those insurance bureaucrats aren't interested in paying out on the behalf of a reckless surgeon.

Why shouldn't health insurance premiums price out similar to car insurance premiums....if I have a lot of accidents where I'm at fault, tickets, DUI, my auto rates would skyrocket.

Yup, it's always a group decision, governed by both private and public bureaucrats, unless you can afford to pay cash from your own pocket. Most of us are not in that position.
