Vast numbers of them never did.
Read.
The Irish and others who settled in the KY coal counties never became educated. They and the corporations destroyed all the land, forests, hills, streams, etc. from over-use. From about 1945 on they have ALL been on the dole. Many are semi-illiterate still (those who stayed).
About 40% left in modern times (1955 onward) and made something of themselves to whatever degree.
Same is true for WV and many other counties here and there....as you know, WV was measured as the dumbest state in the USA.
Much of this is neither right nor left - a combination of factors are involved. But we could have - as a country - decided we wouldn't let the "free market" destroy the places, they might be some great places instead of sad remnants of post-industrialization.

A primer on typical "dole" families in the area (This is MO, but similar deal):

Lots more where that came from.
There are vast numbers of Hasidic Jews who are not assimilated at all - in fact, some started their own towns with their own laws.
As you probably know, Amish don't go in the military. I could go on and on and on, but the USA is full of diversity...and this didn't happen in the last decades.
If you read the 99 cent book you will see that they (Kentuckians) hated America (they hated any country)...hated the government, didn't even have churches, let alone go to them. They had no respect for education or public health, etc.
The book is about relatively modern times - so they had 100+ years to integrate, yet violence and being "on the dole" is where they are. Those who left did become more "American", although they are a lot of the people who lost their jobs in the Rustbelt and are now opioid addicts.
Note - they WERE certainly not scared of hard work. But they didn't pray, say the pledge or respect government. They didn't pay taxes (cheated), etc. etc.
It's good to have some idea what you are talking about.
