...and we have the most expensive health care system in the world. So, then, what exactly are we paying for? Insurance companies and incompetence? Ineffectiveness? Any thoughts as to why we should not have UHC like every other developed nation in the world...?
might be rated the worst but it certainly isn't anywhere near the worst. It might be the best. Why do you think people come from other countries to have treatments? Think of where this rating is coming from for heavens sake.
Not including new treatments, US produces more new medicines than the rest of world combined. In another word, the rest of the world rake in all the health benefits but contribute very little, and US is funding their health care.
Also most "developed world" rely on US to provide national defense - they would all be dead without US.

What people come here for that purpose other than the very well heeled?

While that is an exaggeration, there is a grain of truth in what you say. It is ironic, though, that America is the source of so many advanced medical therapies — both pharmacological and technical — while contributing so overwhelmingly to the diet and lifestyle choices which have brought about the worldwide rise in obesity, diabetes, and opioid addiction.

But, why do we continue paying more for the same drugs sold in other countries?
Same goes for things like joint replacements. Some years ago I was having hip problems and had no medical insurance. When I looked into it total cost in the US would've been in the $45-50K range, the joint itself was over $10K. That very same implant was billed in Belgium at around $1,100 and the operation/hospital could be had in Belgium using a well rated surgeon for less than $20K.

Its not about quality, its about affordability and serving everyone. No poor peasant from Canada or India or China comes here.
With Obamacare people had an option to buy some affordable insurance. But then it has no value if the doctors dont accept it.
If you have a job and employer covered/subsidized healthcare you wont care. Same with Canada or UK, as long its "coming from someone else".
