I guess commercial health insurance rates have been escalating so fast thanks to extremely low Medi-Cal reimbursement rates and nurses unions that now the California Democrats are proposing to cap commercial insurance rates.
The main issue is that California has 12.1 million people on Medicaid and only reimburses at 41 cents for each dollar Medicare pays for primary coverage so healthcare providers have to negotiate very high rates with commercial insurance companies to pay for the unionized nurses payrolls and keep the expensive facilities open.

Projections are that they would have to cut 175,000 positions which would leave the hospitals short-staffed.
The main problem is that California has so many people on Medi-Cal that reimburses primary care doctors only 41 cents for each dollar they get from a Medicare patient.
North Dakota is so rich and has so few people on Medicaid that it can afford to reimburse doctors who see Medicaid patients at 100 cents for each dollar they get from a Medicare patient.
