Plans should be allowed to charge more based on those factors above. Enough extra to cover incremental costs in full, btw.
If there was a magic bullet that could reasonably “ right- size” the US population, the Federal Givernment could absorb the cost of healthcare for everyone and save money.
I have long said that premiums could be based on simple annual waist measures. Those with waists within the generous healthy range would pay the lowest premiums. They are substantially less likely to be diagnosed with Diabetes, Heart Disease, some Campncers and so on.
Give the masses a few years to prepare before implementing.
No exceptions for Medicare, Medicaid, Tricare or any of the other Government programs.
Provide significant financial incentive for people to take more responsibility for their own health.
The free market allowed insurance companies to exclude basic care from their policies and refuse to cover pre-existing conditions. Free market health Care is a failure. What do the Republicans offer as a solution?

If you do some research you will see that we have NEVER had a "free" market when it comes to healthcare....NEVER! For example: One generally could not purchase a policy across state lines even though the costs may be much lower. Just before Obamacare my policy from BC/BS was less than $500/month. The same policy in NJ was nearly double the cost. Don't you think the people of NJ would have liked the option to purchase essentially the same policy for half the cost? By the way, some of the coverage regulations imposed by states were quite bizarre and served only to increase cost to the consumer!! Now, all this said, there does need to be some level of regulation but there also needs to be REAL competition! Republicans know the solution but have proven to be more talk than action

But far less likely to consume very expensive old age health care costs...Its not a given that total health care costs are higher for the overweight group.
You pay too much. I have no monthly cost at all.
Its not universal like yours though. Not everyone in the universe has it.
And why do you have no cost at all? Are you on medicaid? You pay it in the form of lower wages if your employer pays it. And you'll lose it if you get sick and lose your job.
It’s this competition that Democrats fear. Thirty-one Democrats in the Senate are trying to keep people from getting access to expanded popular short-term plans. Democratic politicians in several states and Washington D.C. are blocking these plans and suing to stop association health plans as well.
Competition? That's Republican propaganda nonsense. Single payer health care systems have enormous negotiating power. When you look at lower drug prices that Americans are getting from Canadian pharmaceutical dispensaries, it gives a hint of what the Canadians pay for health care.

Being taller than average I prefer BMI but waist size is probably a better barometer of health. Short people live longer.
I don’t see it happening now that fat shaming is looked down upon.
