The GDP of CA is roughly proportional to its population. Despite the presence of so many of the world's largest companies' headquarter locations, it is not significantly more productive.

I wonder if one real reason it's a "failed" state might be because long ago, California voters decided that they didn't want to pay higher taxes.

That's what happens if it's more important to you to hoard your money, versus paying for a functioning society.

As I understand it California has some of the worst levels of inequality and disparity despite all the taxes.

They did, but it didn't matter because they pay more taxes now than they ever have before.
