There is no requirement to do so.

TO the OP concerning car insurance vs health insurance. No real market for health insurance outside of employers therefore no need to lure people in through incentives or reductions in price.

Your healthy habits are giving the insurance company a discount. Its not like you get those premiums back that you don't spend. We treat health insurance the same as car or home insurance but it is a completely different animal in structure and cost.

Some insurance companies do.

Several of our kids have insurance that offers discounts for regular exercise, improving BMI, ...

Two offer free fitness club memberships.

I believe obesity has a strong mental health component that is rarely addressed by medical doctors. Physicians just don't have the time or experience to discuss a patient's mental health. even if they did ask about mental health, patients are reluctant to admit to feeling depressed because a diagnosis of mental illness has a negative social stigma.

clinical depression is pervasive in America, in my opinion, and a driver in the country's increasing obesity rates.

until the causes of obesity are addressed, obesity rates won't decline even if the obese are punished for their weight by higher insurance premiums.
