It seems the most likely scenario for the use of nuclear weapons is if a nuclear power is overthrown by radicals. Established national governments have more to lose by going nuclear, so they back down. Radicals would sell nukes on the black market to finance their revolution.
Alex Wellerstein is a nerdy, academic cry baby — just check out his blog. The scenario for his nuclear exchange and conflagration is just silly because the circumstances for it became obsolete twenty years ago.
