It was a statement. Read the “question” again. The “idiot” comment was part of the stem. The question was, which president fits the description. But the rest of the description, could only fit Trump as the answer. So the only way a child could get the answer correct, is if he was forced to call Trump and idiot.
Though it would have been wrong to force them to call any of the presidents listed, an idiot, in order to get a better grade.

Donald Trump
Ronald Reagan
Richard Nixon
Jimmy Carter.
They were forced to call Donald Trump an idiot, or else be penalized by getting the question wrong.
I managed to do well on the SATs, MCATs, Medical Boards, Specialty Certification because I know how to read a test question properly.
What you thought was the question could not possibly be, because there was no “all of the above” choice.
Fine. Go on believing what you like, but everyone else who read the article properly, knows I am right.
