The questions are disturbing to a spectacular degree. Dr. Fauci has known since 2005 of the effectiveness of HCQ.....
This means, of course, that Dr. Fauci has known for 15 years that chloroquine and it’s even milder derivative hydroxychloroquine (HCQ) will not only treat a current case of coronavirus (“therapeutic”) but prevent future cases (“prophylactic”). So HCQ functions as both a cure and a vaccine. In other words, it’s a wonder drug for coronavirus. Said Dr. Fauci’s NIH in 2005, “concentrations of 10 μM completely abolished SARS-CoV infection.” Fauci’s researchers add, “chloroquine can effectively reduce the establishment of infection and spread of SARS-CoV.”
Hydroxychloroquine is not a vaccine. Vaccines prime the adaptive immune system of the naive patient to recognize antigens displayed by the virus. Then if the patient is later exposed, they get a rapid and strong adaptive immune response to the infection, squashing it.
The world needs a vaccine, or a miracle drug. No miracle drug is in sight.

And besides the fact that hydroxychloroquine doesn't affect the adaptive immune system, prophylactic use of a drug, any drug, doesn't (normally) provide future benefits.
In this case, what is meant by the term prophylactic use of hydroxychloroquine is that it might be taken even before someone is infected. But first of all, we don't really know how well hydroxychloroquine works for Covid-19 as it hasn't been rigorously evaluated, so there might not be any benefits. Secondly, any such patient would need to continue taking the drug daily, indefinitely, or for so long as the threat was perceived to be there, which would expose them to the side effects for a long time.
Prolonged QT arrhythmia issues are a known side effect of chloroquine and hydroxychloroquine. It's not problematic in most patients, but it can be fatal. If you're going to use it in hundreds of millions of patients - as we do with vaccines, there would be significant mortality from the treatment, unlike with vaccines. That's why we need to wait to see what the efficacy truly is with hydroxychloroquine in proper clinical trials, so that the benefits can be rationally weighed against the risks.
