IDK, plenty of naval accidents over the years that were "self inflicted" for a variety of reasons including poor maintenance and training.
Even elite navies like the US and UK have made deadly errors and the Russians have a knack for sinking their own ships too.

The ship was 44 years old. Probably a malfunction and not to be unexpected. Life expectancy of those ships is about 20 years.

It caught fire during a training exercise. Probably pretty useless as far as modern warfare goes anyway.
