Biden announced 500 million Covid test to be delivered free to your house but you have to go on a website to request it. Is it to get more info on you? Will there be a questionnaire asking your vaccination status in order to get a test kit?

Online is the default way most people order things these days. If they didn't have an online option I'm sure there would be plenty of complaints about that. It is the most efficient way to handle a large number of requests.

The government already knows the vaccination status for most people.
