Can any progressive, try and explain and or defend this ?
Per the article in the OP, it was a form not approved by the school, and that the school immediately told students not to fill out. This is a non-story, being used to try and manufacture outrage by the right wing.

Yes, media on both sides splash this garbage for clicks.
But it did happen, twice apparently.
And I am curious what are the repercussions to this 'teacher'?

Except for the fact that this is the SECOND time this has happened.
"Fool me once...shame on you, Fool me twice...shame on me".
The school warned the teachers in February and apparently they didn't learn. If the school district truly wants to distance itself from this then the teachers need to face disciplinary action that amounts to more than just "Well we will let you off this time, but please don't do it again".
If this was a "Christian" teacher who was using something that wasn't approved, there would be calls from the left for that teacher to be fired with NO "Second chance".
It might be ok for teachers to have some training. It could help them to identify kids with problems.
For any teacher to ask those kinds of questions in a classroom just shows a person who should never be trusted in a classroom. They should be terminated immediately. And so should the principal who told her to cut it out last year. She clearly did not take it seriously. What does that say about the principal?

Regardless, why are so many people responsible for teaching our children doing things like this ? Like others have said, this isn't the first time this has happened.
