"Is the form complete?" is a yes/no question. And the people who stand at the counter to take the forms aren't the ones who manage exceptions to the documentation rules.

Form design is way harder than most think, and getting into subclauses and "Only fill in this is part if under these conditions" lowers the quality of the input.
Just answer "none of your business". Like back when Barry was president and they asked if we had guns in the home.
