We don't have that problem at our library. We don't have a local Walmart, but I have noticed that at any Walmart bathroom we have been to they have a needle drop in the restrooms. They must be the "go to" place for shooting up and restrooms without buying anything.

Recently I had a workman here working on my oven for over an hour with his partner..he was on the floor in my kitchen stressing working on getting the flue cleaned and oven working correctly....long hard job....
As he was leaving he asked if he could use my bathroom....what do I say NO...of course I let him. He did his job and left...stole nothing and did not harm and relieved himself.
It's become such a distrustful country...sad but true. So much fear.
Not only does allowing a stranger into your home put you at risk for rape or murder, but also robbery, or they may steal medications from your medicine chest, or bathroom drawers. With opioid addictions at all time high levels, we don't even keep medications in the bathroom when we have a party.
Unless I have called someone to come to my home, such as a repairman, etc. I would never let a total stranger into my home just because they came to my door. If they haven't called for an appt, or I called them, nope, no way.
Now if I owned a business, I would have a public restroom available. I know what it's like to need one and not be able to find one.
So what if it's a elderly man or a Athelete Jogger or even a High School Boy?
I don't think someone like that is capable of robbing you but of course someone who looks like a Gangster or a Smelly Bum yes they are quite capable

but if a random worker who hadn't already been in your home rang your doorbell and asked to use your bathroom because he was driving by and needed to go, you'd be down with that?

I'm not going to go there, we can let our minds run away with so many scenerios.
"Customers only" is a result of the homeless situation. Allowing them to use the restroom at a business would attract more of them and potentially result in an effect on business from the paying customers. The same for the public restrooms that have pretty much disappeared from Seattle, since they became attractive for drugs and other illegal activities. In our neighborhood many people take long walks around the area. If someone that we were confident was a neighbor who got caught short while on a walk, we would let them use it. The nearest business is over a mile away, and ours do not have any customer only policy, because it's not a problem.
Here in West LA most of the Public Restrooms seem to be that just for "Customers" for non customers it difficult there's a Gasoline Station near where I live but even then you gotta pay for something like gas or buy chips.
Somewhere like Home Depot it's pretty free and open but it can be a drive and hopefully you are near one depending on the area you are at.
I would mostly want to use my own Restroom or a public one before I consider going to someone's house.
Last resort is then finding a bush it gets a little too intense when that many people are difficult with their restrooms.
I would think also there would be a solution for homeless people who don't have a restroom but I guess not.

if he/she sounds nice and calm about it and doesn't look like a theif or creepy possibly.
if it's someone that sounds creepy, rude, and looks like they will murder you then maybe not.
People know if they try to pull something off they know they will get caught eventually especially if there is more than 1 person in the house.
I would doubt someone would want to be causing a crime

Honestly, that's how Ted Bundy operated--he was a nice, pleasant, friendly person, and he asked people for help loading up his car or offered them rides.
