Think it was both.

Anyway.. I've got a friend on FB who has done the same as OP's friend. I worked with them when they were a he.

Doesn't make an attractive woman. My opinion. and yes.. You generally can tell when they're a trans woman because.. Adam's Apple.

But.. They're the same person I was friends with.. Just.. In a different presentation. I have no reason to unfriend them or anything like that..

This particular person, I haven't seen any of the 'followers' like OP describes. Just.. I don't know whether to call them 'encouragers' or what, but every time a pic is posted a few of them will pop in with a 'you look beautiful" or something like that..

Meh. Whatever. I wouldn't particularly agree with a statement like that.. but.. I can understand them giving support and encouragement. I just.. scroll on past.

I am rather curious as to how his wife and 5 kids feel about the whole thing.. That hasn't seemed to have been addressed on the 'ol Facebook page..

That's true.

I think there's just a subset of middle aged men who do this on social media. Sometimes while mindlessly scrolling I'll end up on local news pages. Some of the anchors, who all have a particular "local news personality" look, will post innocuous pictures of themselves at work or at home, (not thirst traps) Most of these women are married with children, but without fail, the comments sections are full of men saying, "you're so pretty/sexy..." It must be so weird for them.

There was a poster here a while back who referred to himself as a "gynephile" which was to say that he was attracted to feminine expression.

I've also known a number of straight cis men who have pondered, perhaps a bit wistfully, what it might be like to be a woman. Some seem to believe that it's a hell of a lot better than what I've experienced, I'll tell ya that much. They have this fantasy of being almost universally loved and desired, of holding a ton of power in all sorts of interactions, of getting constant free benefits of all sorts. They imagine the idea of being objectified as being rather enjoyable. I guess in a fantasy it might be, but in reality...not so much, not for most of us.

Which is also something I've heard expressed by trans women I've talked to, who try to date straight cis men...they feel like they are not seen as human beings if they're just the object of some guy's fantasy. I'm like, "Yeah. Tell me about it." But most of the trans women I know consider themselves to be lesbians and most end up in committed relationships with other trans women, though. I do know one who has leveraged her appeal to the fetishizing cis male sorts, with a lucrative side hustle as a "Fin-Domme." They pay her to humiliate them.

/shrug
Where there is demand...there will be supply...

But if one is wondering what the wives and families of "normal-seeming" straight cis men who follow trans women on IG would think, then hey...hope for the sake of them and probably tons of other guys that their wives and families never see their browser history. Porn search stats show, if nothing else, that people are turned on by taboo concepts. Folks get their mental naughty-buttons pushed in all sorts of ways, this is hardly the strangest out there.

Lots of people have kinks that you would never guess. Some guys are into guys that dress like women, some guys who wear women's clothes still just date women, and mix and match to your heart's content.

A lot of middle aged men are not comfortable with what their sexuality likes, so they live a life with a wife and kids, and try for a little sumpin' sumpin' on the side every once in awhile.

The exotic and forbidden? Fair thought. plain old vanilla sex just doesn't do it for them anymore, perhaps?

Maybe, but I was thinking more along the lines that for their generation, anything other than heterosexual was not socially acceptable. So they did the wife/kids route which is against their sexual nature.

But that drive is strong so they go and seek what they really want on the side.

I've also run into a rather surprising number of men..."manly" men who go on about how "alpha" tough they are and things all the damn time, no less...who secretly wear women's underwear underneath their clothing.

It's a lot more common than anyone might think.

I'm guessing it's in the same mental neighborhood of the high powered business guy who likes to submit in private. Indulgence in the opposite of what he feels he's got to perform in life.

I have mentioned on occasion around here, that for some reason, in person conversation with me seems really comfortable and all sorts of people let their guard down and tell me all sorts of things. Speculation with friends, I am told it's because I am not private or shameful about certain personal matters of my own, and that I come off as non-judgmental and safe. I talk, but also listen well. And I always seem to care what others may be going through.

So yeah. I can't count the number of times that a guy who kept up one persona around just about everyone, pretty readily admitted to me that he had some kind of a secret like that. I missed my calling as a therapist, or perhaps a spy.
